SUMMARY DESCRIPTION OF WORK:
PREFERRED MINIMUM
Qualifications:
1.
Masters Degree in Counseling, Social Work, or Marriage & Family Therapy 2.
2 or more years of experience 3.
License Eligible-LMHP and PLADC 4.
LADC or CADC or substance abuse treatment experience preferred ACCOUNTABILITIES:
1.
Determines whether client is appropriate for agency services; determines needed level of care; refers (internal or external) to appropriate level of care.
2.
Conducts a comprehensive pre-treatment assessment, including 5 axis diagnostic impression and treatment recommendations; refers if other services are needed.
3.
Establishes a treatment plan, with client participation, which specifies treatment structure, outcomes, goals and objectives; within first 2 sessions.
7.
Is dependable and punctual regarding scheduling and attendance.
9.
Maintains relationships with referral sources.
11.
Refers to other Heartland Family Service programs and other community services, as necessary either concurrent with services or for aftercare; follows up to complete referral.
12.
Completes all case documentation for clinical and billing purposes.
13.
Maintains clinical record, meeting established standards of timeliness and completeness.
14.
Is prompt and accurate in completing all required reports, statistics and other requirements.
15.
Displays a courteous and caring attitude at all times to the clientele, volunteers, and visitors of the Agency.
16.
Cooperates and collaborates with program area staff, volunteers, and other Agency staff.
19.
Performs other program related duties as assigned.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
